Transcripts:
Current Students are required to request transcripts in SchooLinks. Directions are provided in Google Classroom for Counseling.
Student athletes will need to First - create an account on the NCAA Eligibility Center website. Then request a transcript through the Eligibility Center. Horizon will receive the request and electronically send your transcript to NCAA.
Students may pick-up personal transcripts from the HHS Records Office. Transcripts WILL NOT be mailed to the student's home, nor faced to a recipient.
Classes 2016-2024:
The former Horizon student will need to contact the HHS Records Office at 602-449-6021 with the following information:

Where do you want it sent or will you pick it up?
Classes 1982-2015:
Request these transcripts from District Office at 602-449-2244 or fax a request to 602-449-2245. Please provide the following (include on fax sheet numbered 1-2-3-4-5):

Name - as it was when you were enrolled at Horizon.

High School Name AND graduation year (or last year attended).

Date of Birth
-
Full address where you would like the transcript sent.

Your signature..... that's what gives the district permission to release the information.
